Évaluation complète et suivi
L’équipe de C-MAVEM propose une évaluation complète : neuroradiologie, neurologie enfant et adulte, neurochirurgie pédiatrique et adulte, ORL, obstétrique, néonatologie, génétique, métabolisme, etc.
L’une des particularités de l’expertise du centre est le diagnostic anténatal et l’appréciation du pronostic des malformations rentrant dans son cadre, les consultations anténatales et la chirurgie materno-fœtale du spina bifida.

Efficacy of intravesical oxybutynin in children with neurogenic bladder dysfunction: A randomized, prospective controlled multi-center trial ;
Malformation of the lumbosacral region (spina bifida) affects the innervation of the bladder in children, leading to a neurological bladder, exposing to incontinence and obstruction to the evacuation of urine. It is responsible for renal failure requiring dialysis and transplantation.
Current therapeutics aim to evacuate urine and reduce intravesical pressure. It gradually combines intermittent catheterization, anticholinergics, botulinum toxin (Botox®) injection into the detrusor (bladder muscle) by cystoscopy and surgery. Oxybutynin relaxes the detrusor, improves continence and reduces intravesical pressure. Intradetrusor Botox® injection is Invasive, has a limited duration of action (6 months) and must be performed under general anesthesia in children.
Schröder et al. (1) performed a randomized study demonstrating the efficacy of intravesical oxybutynin compared to oral administration in adult patients. They found a significant increase in bladder capacity and a significant decrease in side effects in the intravesical oxybutynin group.
Our hypothesis is that this treatment may have a legitimate place in the treatment of neurogenic bladder in patients with failure of anticholinergic treatment before a therapeutic escalation requiring invasive procedures. In this way, we aim to extend the time to therapeutic escalation in the pediatric population.
Primary objective
Compare the efficacy on incontinence of intravesical oxybutynin instillation versus placebo in the treatment of children with neurogenic bladder (spina bifida), performing intermittent catheterization, for whom oral anticholinergic treatment is ineffective.
EQUICHIARI (Promoteur : CHRU NANCY, Investigateur principal : Dr. Stella) :
Intérêt de la posturographie dynamique dans la stratégie de prise en charge de la malformation de Chiari I dans une population pédiatrique.
CONTEXTE : L’étude vise à étudier un des symptômes atypiques, à savoir le contrôle de la posture, dans une population pédiatrique grâce à la posturographie dynamique informatisée (Equitest®, NeuroCom, Clackamas, OR).
PATIENTS et METHODES : les enfants âgés de 6 à 18 ans présentant une malformation de Chiari I confirmée radiologiquement sont inclus dans l’étude. Deux groupes : enfants opérés et ceux qui ne le sont pas. L’indication chirurgicale est basée sur des paramètres neurochirurgicaux standard, et n’est pas influencée par l’étude en cours. Les résultats posturographiques des deux groupes sont comparés ainsi que les résultats préopératoires et postopératoires chez les enfants opérés.
RÉSULTATS : 27 patients ont été inclus dans l’étude ; 10 d’entre eux ont été opérés ; les données postopératoires ne sont disponibles que pour 6. Une différence significative (p < 0,05) a été trouvée lorsque les résultats de la SOT avant et après l’opération sont comparés, mais pas lorsque les enfants opérés et non opérés sont comparés.
CONCLUSIONS : Une différence significative dans les résultats SOT préopératoires et postopératoires a été trouvée, signifiant que la chirurgie de la malformation de Chiari I peut avoir un impact sur le contrôle postural.
